
Dennis Tindall joined the Baxter Regional Medical Center team as a staff physical therapist in June of 1986 after graduating from Duke University’s Physical Therapy Master’s degree program. He has been the Director of Rehab Services since June of 2000. In addition to being a physical therapist, Dennis is also a Certified Athletic Trainer having completed his undergraduate work at Missouri State University. His areas of interest include sports medicine, myofascial release and neurophysiological testing. He has over 24 years of experience in the Physical Therapy and Athletic Training fields.

I graduated from Arkadelphia High School in 2002. I played basketball and volleyball while in school. I attended the University of Central Arkansas in Conway, Arkansas. I graduated from the UCA Athletic Training Education Program in May of 2007. I graduated with a Bachelors of Science degree in Athletic Training. After graduation I gained employment through Cox Health in Springfield, MO. Through Cox Health I was stationed at Evangel University in Northeast Springfield. There I was the Head Athletic Trainer for women’s sports at EU. I have been here at BRMC for 11 months now, I love my job and look forward to growth here at BRMC.

Renee’ Barnes, Physical Therapist
Renee’ Barnes, PT, MS, CLT,CLM has been a licensed physical therapist for 11 years, 10 of those years which she has spent working at Baxter Regional Medical Center. Ms. Barnes has a Bachelor’s degree in Health Sciences, a Master’s degree in Physical Therapy and holds a specialist certification in the treatment and management of Lymphedema through the Klose-Norton Academy. Ms. Barnes has also studied multiple continuing education courses, including manual therapy courses through the International Academy of Orthopaedic Medicine and through Saint Augustine University for Health Sciences in the area of imaging interpretation as it applies to physical therapy. She has also written and received numerous grants through Susan G. Komen for the Cure to fund an outreach program for breast cancer survivors suffering with lymphedema. Ms. Barnes enjoys fishing, hunting, camping, canoeing and otherwise enjoying the outdoors with her family. She also enjoys reading and is an avid fiber artist.

Tammy Burlsworth, Licensed Physical Therapist Assistant, graduated from University of Central Arkansas in 1994 with an Associate in Applied Science Physical Therapist Assisting Degree and has been licensed for 16 years. Tammy is a Certified Lymphedema Therapist from the Norton School of Lymphedema. Tammy has been an employee of Baxter Regional Medical Center for 11 years and states she has enjoyed and been proud to be employed by BRMC. Tammy also is a Certified Healthy Steps Instructor and along with Renee’ Barnes, MSPT and Arlette Michalak, LPTA (they are also certified) teaches a weekly Healthy Steps Class at BRMC. Tammy has been working in the outpatient setting since 2004. Tammy’s children are Christina Lee & Stefanie Jean Morris. Tammy is a Single Adult who is the Single Adult Director at Mountain Home First Assembly of God. “I love the world of physical therapy because I have the pleasure of helping people regain their lifestyles.

Jason Smith, Physical Therapist
I was born in Calico Rock, AR. I was raised near Salem, AR. I attended physical therapy school in Jonesboro, AR at Arkansas State University. I graduated Magna Cum Laude. I have practiced physical therapy for 10 years in a variety of settings including outpatient, inpatient, home health, and skilled nursing home. I have spent the last 7 years at BRMC; the last 5 years in the outpatient clinic. I am NDT trained and I have Lymphedema certification. I am in the process of studying for APTA orthopaedics certification. I am very grateful for the educational opportunities that BRMC provides and for the commitment to customer care that our hospital has made.

Jane Farley, Physical Therapist
Jane Farley joined Baxter Regional Medical Center in June 2009. She is a 1983 graduate of the University of Central Arkansas in Conway with a Bachelor of Science Degree in Physical Therapy. Her 28 years of experience include Outpatient, Acute Care, Home Health, Wound and Burn Care, Inpatient Rehab and Long Term Care Facilities. Her area of expertise is in geriatric rehabilitation having received her NDT certification in 1988. She is also APTA Credentialed as a Clinical Instructor to assist PT interns in achieving their learning experience goals. Jane moved to the Mountain Home area from Paragould, AR. As a memeber of the team she is committed to providing excellent and compassionate care.

Crystal Henry, Occupational Therapist
Crystal Henry is a 1997 Cum Laude graduate of University of Central Arkansas. As an Occupational Therapist with 13 years of experience and board certified as a Certified Hand Therapist, she has spent the majority of her career specializing in the treatment of the upper extremity. She offers specialized physical rehabilitation to those who have pain and limited motion or strength of the upper extremity due to neurological or orthopaedic conditions. John Hilvert, Physical Therapist
John Hilvert, MSPT, graduated from Saint Francis University in Pennsylvania in 1992 with a B.S. in Health Care Administration. He completed his Master’s degree in Physical Therapy at the University of Indianapolis in 1994. John has worked in a variety of therapy settings including inpatient rehab, acute care, and outpatient orthopaedics. John has a special interest in Sports Medicine and is a member of the Sports Medicine team of the Active Lifestyle Institute. John enjoys training for triathalons, coaching local sports teams, his involvement in local church and missions, and spending time with family and friends.

Arlette Michalak, Physical Therapist Assistant
Arlette Michalak has been a physical therapist assistant for the past 10 years joining Baxter Regional Medical Center in April 2001. Her experience includes working in the areas of acute care, home health and currently out-patient physical therapy. Arlette's specialty areas are myofascial release and treating patients with lymphedema. Arlette has lived in the Mountain Home area for the past 17 years coming from the Chicago area with experience in various medical fields. Arlette is proud to be a member of a team where everyone is dedicated to providing excellent, compassionate care.

Keith Kyles, Physical Therapist
Keith is a local who graduated from Yellville-Summit High School in 1989. He attended collage at the University of Central Arkansas in Conway and graduated with a Masters degree in PT in 1994. Keith is currently employed by BRMC as an outpatient PT working in the imaging center. His areas of clinical interest include pre and post surgical orthopaedics and management of spinal conditions. In addition to the outpatient setting he also has experience in acute care, rehabilitation, and home health settings. 2011 will mark his 17th year as a therapist. Keith lives in Yellville with his wife Christy and four daughters. His personal interests include spending time with family and friends, hunting and fishing, and recreational shooting.

Paul Henry, Physical Therapist
Paul was born in Granite City, Illinois in 1960, and lived and worked in St. Louis, MO until late 1994. He is a graduate of St. Louis University Health Sciences program, St. Louis, MO, and received a Bachelor of Science of Physical Therapy in December 1992. Paul worked at SSM Spinal Cord Rehab in Clayton, MO, as a PT Tech and PRN therapist 1991-1994, and as the weekend supervisor for physical therapy at Incarnate Word Hospital, geriatric rehab St. Louis, MO 1994-1994. He also worked in outpatient at Physical Therapy Specialists in St. Louis, MO Orthopaedic specialty clinic and came to BRMC in fall of 1994. Paul is an outpatient therapist at the BRMC Imaging Center Physical Therapy satellite clinic. Paul is married to Crystal (Hunter) Henry OTR/L, and she too is a therapist and they have a one 14 year old son, Alex. Paul loves the outdoors and his hobbies include tasting events (good wine or beer), Dutch Oven cooking, hunting, fishing, shooting, exercise and Green Bay Packers football.

Paul “Richard” Byrum, Physical Therapist
Richard Byrum joined Baxter Regional Medical Center in May 2006 as a staff outpatient Physical Therapist for the Rehab Services Department hospital based outpatient clinic. He left the hospital's on-site outpatient clinic in May of 2009 to facilitate the development of a satellite clinic located in Dr. Thomas Knox's Regional Orthopaedic Healthcare clinic. Prior to moving to Mountain Home, Richard was employed by White County Medical Center in Searcy, Arkansas as an outpatient based P.T. for 5 years and has previously been in private practice for several years, and has also worked in Home Health. Prior to becoming a licensed Physical Therapist, he was science teacher and coach with Blytheville Public Schools. He brings 12 years of experience as a P.T. with his primary experiences and interests in orthopaedics and sports medicine. He has a B.S. in Education from Arkansas State University in Jonesboro, B.S. in Health Science and M.S. in Physical Therapy from the University of Central Arkansas in Conway, Arkansas. He is a certified Clinical Instructor through the American Physical Therapy Association and is currently preparing to become a certified clinical orthopaedic specialist through the APTA and a Certified Strength and Conditioning Specialist from the National Strength and Conditioning Association. I am a native of Arkansas and have been married to my wife Amber for 8 years and we have two boys, J.D and Walker. In my spare time, I enjoy boating, home improvement, kayaking, running, fishing, hiking, sports, church, and spending time with family and friends.

Trena Mayfield, Licensed Physical Therapist Assistant
Trena Mayfield joined Baxter Regional Medical Center in November of 2009 as a Physical Therapist Assistant in outpatient physical therapy. Trena is a new graduate as of May 2009. Trena graduated from Ozarks Technical Community College in Springfield, MO with her degree in the Physical Therapist Assistant program. Trena is committed to learning new skills and growing as a PTA to provide excellent care to every patient every time. I am originally from Joplin, MO. I moved to Springfield, MO to finish college and while in this process I met my husband, Nick. Nick and I have been married for 1 ½ years. We do not have any children but plan on it in the future. We are the parents of a miniature dachshund named Chili, whom we dearly love.

Deborah Wanzor, Occupational Therapist
Deborah “Deb” Wanzor joined BRMC in March of 2006 as a staff occupational therapist for the acute inpatient rehabilitation team. She now is an occupational therapist in the outpatient department where she is focusing on treatment of neurological and orthopaedic disabilities of the upper extremity. She takes pride in assisting these patients to achieve the best possible restoration of prior function. Deb was previously employed by the Dallas, TX Veteran’s Hospital where she worked in a diversified environment of mental health, physical rehabilitation and the upper extremity outpatient clinic. She has been an occupational therapist for 10 years after a 20 year first career as a social worker placing special needs and foreign born children with adoptive families and then foster care supervisor at The Gladney Center for Adoption in Ft. Worth, TX. Deb has a bachelor’s degree from the University of Texas and a Master’s Degree in Occupational Therapy from Texas Woman’s University. Other interests are reading, history, cooking, gardening and a newborn grandson.
